La denuncia non è di qualche movimento anti-psichiatrico, ma dal Dott. Allen Frances del coordinatore della task-force del DSM IV, che sta per essere soppiantato dalla nuova edizione, la quale conterrà molte più malattie mentali classificate.
Frances (team DSM) "Ormai i produttori di droghe legali sono più responsabili delle dipendenze dei produttori di droghe illegali". + 40% per i disturbi bipolari, raddoppiate le diagnosi di iperattività infantile.
Poma (Giù le Mani dai Bambini): "In Italia siamo a rischio con 3 milioni di potenziali nuovi pazienti, non dobbiamo commettere gli errori fatti in USA".
Costa (psichiatra La Sapienza): "Tra le nuove possibili sindromi, il lutto e la dipendenza da caffè: noi medici e specialisti siamo vittima delle mode diagnostiche lanciate dalle multinazionali, attenzione perché è a rischio l'indipendenza della classe medica"
"La semplice tristezza e l’astinenza da caffeina stanno per diventare malattie mentali.
La prossima edizione del manuale, il DSM-V, in uscita nel 2013, potrebbe far diagnosticare come malati mentali milioni di persone sane, affette da normalissimi problemi di tristezza o sofferenza".
La dichiarazione sarebbe normale se rilasciata da un fervente attivista di un movimento anti-psichiatrico, ma diventa eccezionale se consideriamo che è di un "big-boss" della psichiatria americana, Allen Frances, coordinatore del team di specialisti che ha curato l'edizione attualmente in uso del Manuale Diagnostico per le Malattie Mentali, utilizzato per perfezionare diagnosi da psichiatri di tutto il mondo, la cui 5^ revisione vedrà appunto la luce tra meno di 18 mesi.
"Negli ultimi anni abbiamo assistito a una vera e propria inflazione diagnostica – prosegue Frances, che è intervenuto al convegno "Pharmageddon" organizzato presso la Comunità di San Patrignano – e già oggi, ogni anno, il 25% della popolazione statunitense – circa 45 milioni di persone – si vede diagnosticare un disordine mentale, eventualità che sale al 50% degli abitanti se consideriamo le persone anziane.
Nel DSM-IV (l'edizione attualmente in uso del Manuale, curata da Frances, ndr) abbiamo cercato di essere il più cauti possibile ma non abbiamo comunque evitato l’aumento delle patologie e la conseguente tendenza all'incremento delle diagnosi, a cause della quale i disordini bipolari sono 'aumentati' del 40% rispetto a quanto avveniva con la precedente edizione del Manuale (il DSM-III, ndr), quelle di autismo sono cresciute del 25%, e quelle di ADHD, la Sindrome da iperattività e deficit di attenzione dei bambini, sono addirittura raddoppiate, mentre gli antipsicotici sono venduti con un giro d’affari di 50 miliardi di dollari all’anno".
Quella di Frances è una vera confessione-shock, con anche il sapore di un "j'accuse" verso molti Suoi colleghi: "Ormai i produttori di droghe legali sono più responsabili delle dipendenze delle persone rispetto ai produttori di droghe illegali.
Il problema non è nella malafede dei membri della Commissione del DSM – prosegue lo psichiatra – ma nella loro appartenenza all’élite del settore psichiatrico: non si rendono conto che le loro indicazioni, in mano a medici frettolosi e non sempre competenti e con la pressione irresponsabile delle industrie farmaceutiche, possono portare a gravi abusi.
Le nostre attuali conoscenze fra l’altro non ci permettono la prescrizione preventiva degli psicofarmaci, e sarebbe quindi importante che i medici non eseguano le diagnosi con disinvoltura e valorizzino le terapie relazionali rispetto a quelle farmacologiche", ha concluso l'esperto americano.
Sul punto è intervenuto Luca Poma, giornalista e portavoce di "Giù le Mani dai Bambini" (www.giulemanidaibambini.org), il più rappresentativo comitato indipendente per la farmacovigilanza pediatrica nel nostro paese: "La situazione è assai preoccupante, perchè come ha dichiarato sul Corriere della Sera il giornalista Mario Pappagallo 'un mondo di pazzi sarebbe un gran bel mercato', dal momento che solo in Italia ci sarebbero almeno 3 milioni di nuovi potenziali 'pazienti', e non pochi tra loro sono in fascia pediatrica.
Ci renderemo conto a brevissimo – e a spese della salute nostra e dei nostri bambini – di quanto ciò sia assolutamente vero", ha concluso Poma.
Anche Emilia Costa, decana di psichiatria, già titolare della 1^ Cattedra dell'Università "La Sapienza" di Roma e Primario di Psicofarmacologia all'Umberto I°, era nel panel dei relatori di "Pharmageddon", e ha commentato ironicamente: "Dovrei fare istanza al team di colleghi del DSM V affinché inseriscano una nuova patologia, la "bulimia da diagnosi", perchè questo è quello che sta accadendo in America, con influssi concreti anche in Italia: una sistematica medicalizzazione del disagio ad opera di 'inventori di categorie diagnostiche' che sono tra l'altro in palese conflitto d'interessi.
I miei corrispondenti oltreoceano mi dicono che persino un lutto, che è parte della vita di una persona, potrebbe essere diagnosticato come episodio depressivo sul nuovo Manuale, e che tra le patologie che stanno valutando di inserire c'è anche una non meglio precisata 'astinenza da caffeina'.
Tutto ciò è folle, noi medici e specialisti siamo vittime delle mode diagnostiche lanciate dalle multinazionali: attenzione – ha concluso l'esperta italiana – perché è veramente a rischio l'indipendenza della classe medica".
Intanto, il Wall Street Journal annuncia che l'American Academy of Pediatrics ha stilato le nuove linee guida per la diagnosi della contestata Sindrome da deficit di attenzione e iperattività (bambini agitati e distratti), che suggeriscono di consigliare la prescrizione di Ritalin (metilfenidato) anche a bambini in età prescolare, fin dai 4 anni.
Le linee guida americane sono poi recepite in molti paesi del mondo.
